I'm having trouble tuning a 10 speed RD. When I set the H and L limit screws, the tension doesn't allow the shifters to hit the last shift - it only clicks the 9th time. If I undo the L limit screw, the shifter works fine, but throws the RD into the spokes.

When both limit screws are OK, the bottom 4 cogs are good, and the top 4 are good. However, the RD jumps two cogs in the middle with one shift.

Excess friction in the cable housings or shifters can cause problems like this. How old are the cables, housings, and shifters? Dirty? Corroded? Riding in the rain?
Would agree that a bent hanger or derailleur could be the problem.

an update

The derailleur seems to be aligned, and the hanger isn't bent. The RD, cables and housing are all new and haven't been exposed to rain or dirt yet. I've went over it again today, and still only get nine shifts total - it still jumps two cogs in the middle.
