Doing a drivetrain swap from one bike to another. Install went fine, but I couldn't get the rear derailleur dialed in. Barrel adjustment, limit screws, different lengths of cable housing for the curve radius... all no go. I could get sections of the cassette (21/24/27 or 12/13/14) to shift smooth, but other sections would then be just a bit out of alignment.

Finally, after spending way too much time at the back end of the bike, it occurred to me that the problem might not be with the derailleur, but with the shifter. I sprayed some WD-40 into the levers, worked it in, and viola! Shifts like butter through the whole cassette. I think just enough gunk had built up that each click of the shifter wasn't pulling the cable as crisply as it should.

Just thought I'd share my embarrassment in case anyone else has a similar problem.
