should be parallel to the chainrings and 2-3mm above the top of the teeth on the big ring.

My experience has been that the closer you get the outer FD cage to the teeth of the big ring, the better the control of shifting, particularly from a "shifting off the chain rings" standpoint. I adjust FD so that they just clear the big ring. YMMV
