I just picked up a used frame, the steerer tube is cut to short. Is it possible to replace the tube? Or do I need to replace the fork and just buy one uncut.

In practice, there is no way a user can replace the steerer tube on a fork. You need to buy a new one.

stem extender

this might work. stack your spacer rings underneath your stem. you may need to make your own shim. nashbar/performance sells them. The top cap my not apply any downward pressure but if you use the top cap from your stem and the long bolt from the stem it could work.
