I had Susan do some custom decals for me.... they're great.

For your name you can always buy the transfer letters and clear coat from Business Depot or art supply store.

Take a look at www.bikenames.com. I haven't ordered anything from them so I don't know first hand how thier product/service is, but others have recommended them. Also, www.longscycle.com and www.vcgraphix.com

I do a lot of triathlons and I always like how people have stickers with their names attached to their frames. Any idea where I can get a decent one?

I got some from Long's Bicycle Supply a few years ago. I assume they still do this. They had script in white and several colors in block letters.

I get mine from a local instant sign company. They're laser cut stick ons. They have lots of fonts and colors and, for a price, they can scan almost anything into their computer and make a custom stick-on for you.
