I am looking to replace the brake pads on my new Cannondale Six13 1. I am going to up grade to Kool Stop Salmon pads. My question is do I need to buy the holders or just the pads and swap the pads out? Anyone know were to get them online for a good price?

Same question, but for SRAM Force/Rival pad carriers. I'm trying to figure out what I need, the Supra pad only, or the pad and carrier.
The supra has no "carrier"; the pad and hardware are all one unit.

So they will certainly work. Koolstop also makes pad-only inserts compatible with Ultegra/DuraAce, but I don't know it they're compatible with the Sram, or with whatever Cannondale uses. I suspect they are, but personally I'd upgrade to the the Koolstop hardware (either the Supra one-piece or the Dura holder-plus-pad; it's better hardware -- more adjustable than the Shimano shoes.

Harris Cyclery has them all -- I don't know how their prices compare with others.

Best brake upgrade, Kool Stop pads

As JCavilia says, you can get the pads in Shimano cartridge style. They are called "Dura-type" in Kool Stop lingo. They will fit Ultegra/Dura-Ace brakes from the 9spd series onward. They will also fit the Mavic SSC brakes. Not sure about SRAM. Kool Stop also has Shimano-style holders with pads, these holders are a nice upgrade from Shimano because they have orbital washers that allow the pad to be set perfectly parallel to the rim. They also come with both salmon and mixed compound pads. Here is a good online source, no affiliation except that I am a satisfied customer.

I've seen some nasty red and orange marks that the coolstops leave on the rims. Don't be surprised to see some ugly marks on those prized rimsets. Just FYI
