wedges and arch support/insoles would be my first suggestion. They have made a big difference for me..comfort, pain reduction, and better performance. Get a fit done, or do some experimentation.
Another thing to consider is toe box width. My suggestion is that it happens because I put more pressure on the outward part of my foot when pedalling. Could I be right? Can I cure it by tilting my shoe with a small wedge under the left cleat?
