These are the notes I made for the bike shop. They cant figure it out. Sure would appreciate some help.
WHEN IN THE SMALL CHAIN RING

JUMPS FROM 5TH TO FORTH GEAR

WHEN I TIGHTEN THE CABLE OR TURN 1/4 CLOCKWISE TI WONT GO ONTO HIGHEST GEAR

CHANGING FORM SMAL CHAINRING TO LARGE, IT HAS JUMPED LARGE CHAINRING

WHEN IN 5TH GEAR ON REAR IT JUMPS DOWN TO FORTH GEAR
USUALLY IN SMALL CHAIN RING, AOMWTIMES IN LARGER GEARS ALSO IN SMALL CHAINRING

WONT SHIFT FROM SMALL CHAINRING TO LARGER CHAINRING UNDER LOAD
OR GOING UP HILL ,COULD IT BE BECAUSE OF FLEXABILITY OF FRAME??

IT JUMP DOWN A GEAR OR UP A GEAR UNDER LOAD--GOING UP HILL

MEASURE D CHAIN AND IT SEEMS FINE.

If you follow the instructions closely and in proper sequence you should be able to correct the problem, unless there has been damage to any of the components. You can find the same instructions at the Shimano site.
Your problem is most likely to be shift cable stretch and can be corrected with the barrel adjuster found at the rear derailleur.
