1) Yes. I am using a rear Shimano XT skewer on my road bike.
2) Some brands are better than others - or to be more accurate, some more tricky to set up than others. And that's just dumb.

If the skewer is longer than it needs to be cut the extra off. The thickness of the bike's dropouts will have as much effect as the hub spacing.

Yes. The exception are the newer Thru-axles used on many newer MTB's. But yes, MTB QR hubs are 135mm rear (100mm front)

titanium for a 200 lber... may be too heavy for it (eating into the designed safety margin)
